Community Health Centre – 209 Mavety St.

We are expanding to meet the growing needs of the communities we serve. The new site will improve access to care, reduce wait times for people who are looking for primary care, and allow us to continue our commitment to improving access for our existing clients.

The new site will open in the fall of 2026.

Yes. All current locations will remain open and continue operating as usual.

The site is located at 209 Mavety Street in The Junction and is easily accessible by public transit. Several TTC routes serve the area, including the 26 Dupont, 40 Junction, 41 Keele and 89 Weston buses.
Additional information, including maps, parking and accessibility details, will be provided closer to opening.

Our hours remain unchanged across our sites.
Monday to Thursday: 8:00 a.m. to 8:00 p.m.
Friday: 8:00 a.m. to 5:00 p.m.

Yes. All existing locations will remain open.

The opening of the Mavety site will not affect your relationship with your current provider.

Clients will not be automatically reassigned to the Mavety site and will continue to have access to their current provider. If appropriate, options will be discussed with you about accessing appointments at the Mavety site.
Through our team-based, interprofessional approach to care, clients may also have opportunities to access services across sites to ensure timely access to care.

Where possible, clients will have options based on service availability and their care needs.

Yes. CommunitiCare Health provides care using a team-based model, and access to your health history is important to ensure you receive the best possible care.
Records will continue to be shared securely within your circle of care in accordance with the Personal Health Information Protection Act (PHIPA) to support continuity of care. As is our current practice, records are only accessed by staff involved in providing your care, and only when needed.

There are currently no planned reductions to services at existing locations.
Our goal is to provide services and programs that meet the needs of our clients and communities, and we will continue to adapt and respond to changing needs over time.

The new site will provide interprofessional care, with different providers working together to support clients. It will offer the same primary care services currently available at our existing locations.
We will continue to ensure that our services and programs evolve to meet the changing needs of the community. While many services will be available when the site opens, there will also be opportunities for future growth and development.

We are committed to ensuring a smooth transition with minimal disruption to services as we open the Mavety site.
Careful planning will be required to coordinate training activities for our staff and appointment scheduling. Temporary locum coverage may also be used for short periods if needed to ensure continuity of care.

The site was selected based on identified community needs, gaps in access to care and opportunities to better serve local populations. In 2014, the Junction Commons Project, a local working group, conducted a feasibility study to explore repurposing the vacant police station at 209 Mavety St. into a community asset. Through community engagement, the group identified key needs in the Junction neighbourhood, including:
• Seniors services
• Health and social services
• Food and nutrition programs
• Arts, learning, and development opportunities
After reviewing the study, the Junction Commons Project endorsed CommunitiCare Health (formerly The Four Villages Community Health Centre) as the ideal partner to fulfill these needs. The City of Toronto awarded CommunitiCare Health the 209 Mavety St. site to bring this vision to life.

CommunitiCare Health will continue to provide team-based primary care and health promotion programs at the new site. We will continue serving the people and communities we have supported for many years and will build on these services as community needs change.

The new site is expected to allow us to serve more people and improve access to primary care. Over time, this may help reduce wait times for people who are looking for a primary care provider.

We value our existing partnerships and expect them to continue.
The opening of the Mavety site will not negatively affect current partnerships. As community and client needs evolve, we will continue to look for opportunities to strengthen existing partnerships and explore new collaborations that support the people and communities we serve.
