Our Services

We provide affordable supportive housing and case management services for adults with serious mental health and/or addiction issues. Our housing supports include congregate living, high support living, addictions supportive housing and independent living.

Case Management services are available to individuals in our supportive housing programs as well as those living in the community independently.

Seniors Social Group

The Seniors Social Group provides a friendly mutual aid and self-help group for people who may identify as socially isolated, coping with chronic pain, and mental health challenges. The group relies on core members to provide leadership in teaching crafting skills, peer support, and sharing information and resources to all participants. Adult Day Program

The program offers a variety of quality activities and services in a safe and supportive environment to individuals living in the community who are physically frail or have cognitive impairment. Participants receive nutritious meals and snacks, and assistance with activities of daily living. Operation is Monday to Friday 9:30 am to 2:30 pm. There is a daily fee for service. Transportation to and from the program is also available for an additional fee.

Fees:

$25 per day
$5 round trip transportation

Yoga 50+

This weekly in-person program is for anyone who wants to relax and rejuvenate with a guided meditation and breathing exercise with gentle yoga on chair.

Mindfulness in Motion

This virtual program is based on mindfulness practices, to help develop capacity and awareness to be fully present in our lives by continually using practical skills to increase coping. There will be weekly homework to assist you with your home practice. This workshop is for adults experiencing chronic conditions/stress; a minimum familiarity with mindfulness is strongly recommended.

Mindfulness & Expressive Arts

Tune into what is happening now, both within yourself and around you. This 8-week in-person program will teach you how to be more present and pay attention to everyday life, cope with negative emotions and meditate. The program will also incorporate art based therapeutic techniques.

Long Covid

Join us for a virtual Long Covid Group, meeting monthly. This group is a self help supportive and compassionate environment aided by healthcare providers and your peers regarding lingering symptoms from a Covid-19 infection. Group education on various health topics will be provided, including managing fatigue, coping with mental health implications, tips for returning to your everyday activities, breathing techniques, exercise, and relaxation strategies. This group will be facilitated by a Therapist and an Occupational Therapist.

Boosting Brain Health and Memory

This 6-week in-person program at our Lakeshore location is aimed at supporting older adults in learning ways to boost overall brain health and memory. Each session will include an educational presentation and an interactive workshop to practice memory strategies in a supportive environment.

Healthy Parents, Healthy Babies (p.m. session for pregnant to 6 months)

This weekly in-person or virtual program is for pregnant women and women with newborns who are isolated, new to Canada or have financial difficulties. Enjoy weekly presentations about child safety, child development, breastfeeding, baby care, nutrition, cooking and more. The programs is staffed by a Registered Nurse, Registered Dietitian and Community Health Worker, and is designed to support women through pregnancy as well as baby’s first year.

Food Bank Mondays

The Daily Bread Food Bank, Second Harvest and CommunitiCare Health have teamed up to help provide food for anyone in need during these difficult times.

Every Monday from 5 to 7 pm at CommunitiCare Health, 3446 Dundas Street West.
