About Our Amalgamation

Regeneration Community Services, Storefront Humber Inc. and The Four Villages Community Health Centre have amalgamated.  We are a single agency for community health care and support services.  Our three organizations are now one entity under the name of CommunitiCare Health.

Our community health agency now provides our clients and communities with the greatest range of community health and support services. This makes us a distinctive agency in Toronto offering a unique blend of community support, community mental health and addictions services, supportive housing, primary health care and home care.

Why the Three of Us?

Because our services are complementary and help meet our clients’ needs to be healthy and stay in the community:

• Regeneration Community Services provided case management services, supportive housing and peer support programs for people living with mental health and addictions 
• Storefront Humber offered a broad range of community support services to seniors and adults living with disabilities. Storefront is also a homecare service provider organization with Home and Community Care Support Services
• The Four Villages Community Health Centre provided health care services and health promotion programs through an inter-professional, team-based primary health care model

We will work to realize a common vision: a single point of access for a full range of community health care and support services. These services can help keep our clients physically and mentally healthy and living at home.
