Our focus is on keeping you healthy, not just treating you when you are sick. We offer a team of health care providers who work together to care for your health.
How to Access Services
We offer a wide variety of services. Some of our services have waitlists, like Doctors and Nurse Practitioners, and some need a referral or assessment like Home Care, Physiotherapy or Mental Health support. Most of our Primary Health Care services are provided from two of our locations: 1700 Bloor St. W (416) 604-3361 and 3446 Dundas St. W (416) 604-3362. If you are already a registered client with us, please contact our Bloor or Dundas location to book or change appointments.

After Hours Statement
If you see a doctor or nurse practitioner at our clinic and have an urgent health concern that cannot wait until the clinic opens, you can call our after-hours phone service for medical advice.

One of our doctors will call back within 60 minutes to offer advice over the phone.
In an emergency go to your nearest hospital ER, or call 911.
Services

Medical Care
We offer care with our team of Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses and Clinical Assistant for your primary care needs. These services often have a waiting list. Please contact intake for more information.

Rehabilitation
Our Physiotherapists, Occupational Therapist and Kinesiologists help with conditions or injuries so you can get back to your regular activities with less discomfort. A referral is required if you are not a client with us.

Nutrition Care
Our Registered Dietitians are trained to help people understand the role food and healthy eating have in staying healthy and preventing disease. They work with you to help you feel your best with nutrition! A referral is required if you are not a client with us.

Counseling and Supports
Our Therapists and Social Workers provide one-on-one counselling and facilitate therapeutic group programs. Our Community Health Workers help people through many different life challenges by offering individual and group support. Currently, you must be a client with us to access this service.

Footcare
Our Chiropodists assess, treat and care for feet. A referral is required if you are not a client with us.

Diabetes Education
In partnership with West Toronto Diabetes Education program, a division of LAMP Community Health Centre, we offer support from a Registered Nurse, a Registered Dietitian and footcare for people with Type 2 Diabetes or those who are at risk of developing Type 2 Diabetes.
