Physiotherapist

As the physiotherapist, you will provide care primarily in-person, at various on-site and off-site locations in the community. You will have the capacity to provide virtual care to meet client needs, as required. You will work independently and as part of an inter-professional team, from a framework that is trauma-informed, with a focus on anti-black and anti-indigenous racism, discrimination and oppression. Care practices also include a focus on the key determinants of health, with an emphasis on disease prevention, empowerment, health promotion and community capacity building. Vice President, Corporate Services

As part of the executive leadership team, the Vice President of Corporate Services leads and enables a team of administrative staff working at multiple sites providing the full range of “back office” corporate services to support effective and efficient delivery of health services/programs and support services and to meet other corporate organizational needs contributing to organizational achievement of Quadruple Aim operational excellence.

Working in collaboration with a VP Programs, Program Services and Supports (PSS), a Chief Information Officer and having the active support from a Chief Financial Officer and Director of Human Resources and other key staff, this role provides active leadership, regular guidance, oversight and accountability for day-to-day delivery of all corporate services. This includes the agency’s Finances, Accounts Receivable/Payable and annual Audit, Supply Chain, Human Resources in a Unionized Environment, Joint Occupational Health and Safety, Property & Facilities Management/Maintenance and Real Estate Acquisitions and Leasing functions, and, supporting the CIO with IT/IM and working with the VP PSS on Decision-Support and Integrated Risk Management.

In addition to responsibility for and oversight of day-to-day corporate services, the role includes ensuring that each back-office department remains compliant with all applicable laws, legislation, and internal policies, fosters the creation of a collaborative “back-office team” with back-office functions integrated in terms of its people, processes and technology.

Residential Support Worker

Residential Support Workers are responsible for providing a range of services to clients, consistent with the values and goals of the program/agency. They provide hands-on assistance to address the physical/cognitive, mental health and addictions needs of clients. RSWs are required to provide support to clients with daily living activities and assist with shopping, food preparation, cooking, food serving, and client supervision. This position requires that individuals assess potential crisis situations and intervene accordingly, while maintaining established routines of the program including all security, fire, general health and safety procedures. Residential Support Workers facilitate the overall smooth running of the program and building, consistent with the philosophies of the agency and under the direction of the Program Manager.

Primary Health Care Coordinator

Temporary Part-Time Primary Health Care Coordinator

This is an innovative role seeking to evolve the way in which Primary Health Care Services are provided in Ontario through streamlined and coordinated care with the Ontario Health at Home. The Primary Health Care Coordinator actively collaborates with clients, caregivers, primary care, and allied providers within the organization and with external partners. This position will develop and execute coordinated care plans (CCP) based on client care goals to improve the care/supports for those clients with complex health needs to help maintain their well-being and independence in the community. The Primary Health Care Coordinator, with the assistance of Community Health Workers, mental health case managers and Toronto Central LHIN home and community care staff for wider system navigation, supports and coaches clients and their families/support people in actively coordinating care within Primary Health Care as well as externally with other agencies. In addition, the Primary Health Care Coordinator creates formalized care teams consisting of multiple types of providers both internal and external to CommunitiCare Health. The Primary Health Care Coordinator also functions as a liaison between internal and external service providers, specialty medical clinics and the client and their family/support person.
The Primary Health Care Coordinator works within a framework that appreciates the impact of the social determinants on wellbeing and health as well as places an active emphasis on disease prevention and health promotion.

Peer Support Worker

The successful candidate will be responsible for supporting and facilitating the clients’ unique wellness and recovery goals through group activities as well as engaging clients on a one-on-one basis. The Peer Support Worker will utilize their lived experience within the Mental Health Community to help assist, encourage, and mentor the clients through their own personal journey of recovery. The purpose of this role is to embark on a mutual learning experience with the clients we serve, rather than assuming the role of a counselor or therapist. The Peer Support Worker will be consistent with the philosophies of the agency and peer program and showcase excellence in the delivery of peer support competencies; complimented with compassionate and client-centred forms of engagement. The successful candidate will be part of the Peer Support Team and work closely with the staff team at each site.
