Social Worker/Therapist

As the social worker/therapist, you will provide counseling/therapy, case management, client advocacy and group work for children, youth, and adults, on and off-site, virtually and in-person, including homes, schools, shelters, and other community locations. You will work independently and as part of an interdisciplinary team, from framework that is trauma-informed, with a focus on anti-black and indigenous racism, discrimination, and oppression. Care practices also include a focus on the key determinants of health, with an emphasis on prevention, empowerment, health promotion and community capacity building. This is a unionized position.
Peer Support Worker
Permanent Part-Time Peer Support Worker
The successful candidate will be responsible for supporting and facilitating the clients’ unique wellness and recovery goals through group activities as well as engaging clients on a one-on-one basis. The Peer Support Worker will utilize their lived experience within the Mental Health Community to help assist, encourage, and mentor the clients through their own personal journey of recovery. The purpose of this role is to embark on a mutual learning experience with the clients we serve, rather than assuming the role of a counselor or therapist. The Peer Support Worker will be consistent with the philosophies of the agency and peer program and showcase excellence in the delivery of peer support competencies; complimented with compassionate and client-centred forms of engagement. The successful candidate will be part of the Peer Support Team and work closely with the staff team at each site.
Chiropodist

Performs assessment, diagnosis, treatment, health education/counselling for diseases, disorders and dysfunctions of the foot and other clinical foot care activities according to the College of Chiropodists of Ontario.
Occupational Therapist

As a member of the inter-professional primary health care team, the Occupational Therapist provides primary health care to individuals, families and the community, and participates in the development and implementation of quality improvement initiatives with a focus on excellence and coordination of care. This is a unionized position.
Residential Support Worker
Temporary Part-Time
Residential Support Workers are responsible for providing a range of services to clients, consistent with the values and goals of the program/agency. They provide hands-on assistance to address the physical/cognitive, mental health and addictions needs of clients. RSWs are required to provide support to clients with daily living activities and assist with shopping, food preparation, cooking, food serving, and client supervision. This position requires that individuals assess potential crisis situations and intervene accordingly, while maintaining established routines of the program including all security, fire, general health and safety procedures. Residential Support Workers facilitate the overall smooth running of the program and building, consistent with the philosophies of the agency and under the direction of the Program Manager.
Housekeeping Worker
Permanent Part-Time
Housekeeping workers are responsible, under the supervision of the site Program Supervisor, for cleaning of all areas of the residence. The responsibilities include washing windows, cleaning washrooms, dusting, vacuuming, sweeping, washing, waxing and polishing floors, as well as other cleaning duties as assigned by the Program Supervisor, in accordance with the maintenance standards.

RN/RPN Care Coordinator

The Care Coordinator, under the direction of the Director, Community Programs is accountable and responsible for assessing client needs, determining eligibility of service, developing an appropriate plan of care and monitoring the care delivered. Drawing on your knowledge of health and social services, you will assist clients to navigate the health care system. You will work with clients to facilitate linkages with the right information and help them understand and manage their short-and long-term goals.
Physiotherapist
As the physiotherapist, you will provide care primarily in-person, at various on-site and off-site locations in the community. You will have the capacity to provide virtual care to meet client needs, a required. You will work independently and as part of an interdisciplinary team, from a framework that is trauma-informed, with a focus on anti-black and anti-indigenous racism, discrimination and oppression. Care practices also include a focus on the key determinants of health, with an emphasis on disease prevention, empowerment, health promotion and community capacity building. This is a unionized position.
