We have launched a Client Feedback Tool (Compliments, Concerns & Suggestions) to make it easier for clients, patients, and families to share their experiences with our services.

Listening to feedback helps us learn what is working, understand where we can improve, and recognize the work happening across our teams every day. Your input helps us continue to provide care that meets the needs of our community.


How You Can Share Feedback

You can use our online feedback form to share:

  • Compliments about a staff member, team, or service
  • Concerns about your experience
  • Suggestions that could help us improve

The form is simple to complete and works on both mobile devices and desktop computers.

You may also choose to submit feedback anonymously if you prefer.


Prefer to Speak With Someone?

The digital feedback tool is one way to share your experience, but it is not the only option.

If you would prefer to speak directly with someone about a concern or question, you are always welcome to do so. Our staff can help connect you with the appropriate person 416-855-1066.


What Happens With Your Feedback

All feedback is reviewed by leadership within the relevant service area and helps inform our quality improvement work.

We also share positive feedback with staff and teams so we can recognize the care and support happening across our organization.
